One strong aspect of disease that it brings on a state of dissociation. One is not completely in the here and now anymore, but involved with the problem. The problem attracts the attention, leading one away from the here and now. The concentration is diminished, one makes mistakes, is distracted, can go to daydreaming and have persistent thoughts on the problem or related subjects.
